Why Doctor is S-tier in Maple Hospital
Doctor earns S-tier for two concrete reasons beyond XP rate:
Exclusive area access
OR and Anesthesia bay are Doctor/Surgeon-only. No other free role gets into these areas. If you care about map exploration and RP variety, Doctor is the gate to the most restricted hospital zones.
Deepest non-Gamepass RP
Doctor's interaction menu has more options than Nurse — diagnosis, prescribing, surgical referral, consulting. Players in the Patient role engage differently with a Doctor character. The conversation depth is noticeably higher in my sessions.
The Doctor role lives or dies by correct sequencing. Use the Diagnosis Wizard to select your patient symptoms and get the exact treatment order, department, medication, and XP for every case type.
Doctor specializations — all 3 paths
General Practitioner (GP)
Main ward + consultation rooms
Unlock: Level 10 (base)
Best for: Versatile all-rounder — highest area access of any non-spec Doctor
GP is what you get at Level 10. The GP kit includes diagnosis tools, prescription writing, and direct patient consultation that Nurses can't perform. In my testing, GP in a busy main ward gives XP comparable to Nurse — the difference is the roleplay complexity is significantly higher. Patients engage differently with a Doctor than with a Nurse.
Anesthesiologist
Operating Room + Anesthesia bay (exclusive)
Unlock: Doctor + level progression
Best for: Highest XP in the Doctor tree — surgical procedures give premium XP
Anesthesiologist unlocks the Anesthesia bay and OR access for pre-surgical care. The XP rate here is the highest I measured in the Doctor tree — surgical procedures seem to give a premium XP payout compared to standard consultations. The Anesthesia bay also has fewer competing staff than the main ward.
Hospitalist
Long-term care ward
Unlock: Doctor + level progression
Best for: Low-intensity RP — monitoring, rounds, patient documentation
Hospitalist is the Doctor equivalent of CNA for Nurse — lower intensity, consistent small XP ticks from patient rounds. It's useful for longer sessions where you want to stay engaged without high-intensity treatment sprints. The long-term care ward has a calmer atmosphere than the main floor.
When to switch from Nurse to Doctor
This is the question I get most often. My data-based answer:
Switch at Level 10 if...
- ✓ You want OR and Anesthesia bay access ASAP
- ✓ RP depth matters more than XP rate
- ✓ You plan to go Surgeon next (Level 20)
- ✓ You're playing with Doctor/Patient RP partners
Stay Nurse until Level 25 if...
- → Your goal is reaching Level 30 (Vet) ASAP
- → You want the L&D XP bonus (~15% better to Level 30)
- → You're within 2-3 sessions of Level 25
- → Nurse specializations are your main interest
Bottom line: both decisions are valid. The XP difference between switching at 10 vs 25 is small enough that personal preference should decide. I personally switch at Level 10 because the OR access is worth it to me — but I've run the numbers both ways and the gap is around 8-12% total XP by Level 30.

How We Tested Doctor Data
XP rates for Doctor specializations come from timed 15-minute sessions across 3 server sizes, the same methodology used for the Nurse guide. Doctor XP was measured after switching from Nurse at Level 10 (fresh Doctor, no specialization bonus yet) and again after unlocking Anesthesiologist. The Nurse vs Doctor switching analysis is based on tracking total XP at Level 10, 15, 20, 25, and 30 across two separate playthroughs with different switching strategies. Last verified: May 13, 2026.

Level 10. Play as Nurse from Level 0 to reach it fastest — Nurse gives 8-12 XP/min with active play.
What specializations does Doctor have?
Three: General Practitioner (base), Anesthesiologist (OR + Anesthesia bay), and Hospitalist (long-term care). Anesthesiologist gives the highest XP rate (9-13 XP/min).
Is Doctor better than Nurse for XP?
Similar. Nurse peaks at 12 XP/min with L&D. Doctor Anesthesiologist peaks at 13 XP/min. The difference is small — RP access is the real separator.
Should I switch to Doctor at Level 10 or stay Nurse?
Switch at 10 for OR access. Stay Nurse until 25 if you're chasing the L&D XP bonus or heading straight to Level 30 Vet. Both paths are valid.
What areas does Doctor unlock?
Operating Room (OR) and Anesthesia bay — both exclusive to Doctor and Surgeon. Doctor can also enter the Maternity wing for consultations.
