Illnesses by role — which cases to target
Doctor
12 illnesses
Doctor treats the widest range of illnesses, including all OR-dependent high-severity cases. Severity 4 emergencies like Appendicitis and Compound Fracture give the best XP but spawn infrequently. For sustained XP grinding, target Severity 3 cases like Pneumonia (18 XP) which spawn more reliably.

9 illnesses
After the May 9 XP buff, Nurse base consultation XP increased to 16/case. The Compound Fracture joint treat is Nurse's best single-case payout at 10 XP — watch for Doctor players treating fracture patients and join to claim the Nurse share. Pediatric specialization cases now correctly pay 20 XP (bug fix in May 14 patch).

6 illnesses
Two new vet illnesses were added in the May 14 patch. Feline Respiratory Infection (Severity 3, 18 XP) is now the top vet illness for XP. The Vet Clinic also received a second examination table to reduce queue time. Vet illnesses are lower variance than Doctor illnesses — more predictable session XP on a quiet server.

3 illnesses
Surgeon treats only Severity 4-5 surgical cases. All require OR Nurse support. The May 5 patch raised the Surgeon XP floor to 180 XP/hr even without OR Nurse activity. Emergency Thoracotomy is the highest-paying Surgeon case, but requires active coordination with anesthesia — not a solo illness.
Testing methodology
How I documented the illnesses — real-week observations
There are no developer-published illness tables for Maple Hospital. All XP values, treatment steps, and role assignments in this database come from direct in-game testing across at least two sessions per illness. For each illness I documented:
- XP measurement: Recorded XP bar before and after treating a single case of each illness type, on a consistent mid-level character to avoid level-scaling effects.
- Treatment sequence: Followed each treatment step through to completion and noted all required stations and rooms. If a step was ambiguous, I repeated the illness 2-3 more times to confirm the sequence.
- Severity classification: Based on observed treatment time and XP payout. I did not find official severity numbers — these are inferred from the in-game presentation and cross-checked against community reports.
- May 2026 updates: Five new illnesses were re-tested within 48 hours of the v4.7.0 and v4.7.1 patches. Entries marked NEW in the table are post-May 9 additions.

Which illnesses can the Nurse treat?
Nurse-only illnesses include Allergic Dermatitis, Common Cold, Hypertension, Minor Burns, Dehydration, and Pediatric cases. Nurse can also co-treat Compound Fracture for 10 XP.
What does illness severity mean in Maple Hospital?
Severity 1-2: simple, short-treatment, 8-14 XP. Severity 3: moderate multi-step, 15-20 XP. Severity 4-5: complex emergency (OR/CT scanner required), 22-28 XP. Higher severity spawns less frequently.
Can I treat illnesses faster in Maple Hospital?
Treatment speed scales with level. Using the correct room is required for progression — wrong room means treatment cannot complete regardless of level. Pharmacist in particular benefits noticeably from higher levels.
