What to do in your first 30 minutes, how the patient status system works, and the fastest XP route as a free-to-play Nurse — all based on in-game testing in May 2026.

TL;DR

Pick Nurse (free, Level 0). Treat Critical and Serious patients first — they give 2× more XP than Stable cases. Unlock Doctor at Level 10 as your second role. The game is fully playable as free-to-play — you don't need Robux to reach any main role unlock.


Your first 30 minutes

A step-by-step plan based on 5 full play sessions as a new player.

0–5 min
Choose Nurse and find the General Ward
Nurse is free and earns the best XP for a new player. Head to the General Ward — the central patient treatment area. Don't worry about specializations yet.
5–15 min
Treat Critical and Serious patients first
Look for red and orange status indicators. These yield the most XP. Critical patients need immediate action; Serious ones give you about 2 minutes. Ignore Stable and Recovering patients for now — other staff will handle them.
15–20 min
Complete 8–10 patient cycles
After 8–10 successful treatments you'll have a feel for the treatment flow and should be close to Level 3–5. At Level 5, the first Nurse specialization (Pediatric Nurse) unlocks.
20–25 min
Check the Codes menu
Open the reward/codes menu and try any active codes. Even a small Gems boost at this stage compounds over your first hour.
25–30 min
Plan your path to Level 10
Doctor unlocks at Level 10. Keep treating Critical and Serious patients as your primary XP source. On a busy server you can reach Level 10 in 2–3 focused sessions.

Patient status system

Six status types determine patient priority. Higher urgency = more XP.

Critical

Immediate treatment required. Yields the highest XP of any patient type. Prioritise these on busy servers.

Serious

Treat within ~2 minutes. Second-highest XP. Good to balance with Critical cases.

Moderate

Standard treatment queue. Reliable baseline XP — most patients will be Moderate.

Stable

Low urgency. Still earns XP, but less than higher-priority cases.

Recovering

Treatment is in progress. Wait for it to complete before reassigning.

Discharged

Patient has been successfully treated. No further action needed.


Fastest XP tips

Five habits that noticeably speed up progression — tested across 40+ hours.

Join busy servers
More patients = more XP per session. Server with 10+ active players is noticeably faster than a half-empty one.
Prioritise high-urgency patients
Critical patients give roughly 2× the XP of Stable patients. Always treat the highest-priority available case.
Don't idle
XP accrues from completed patient treatments, not time in-server. Idle sessions earn nearly nothing.
Stay in your role
Switching roles mid-session can interrupt ongoing treatment chains. Commit to one role per session for smoother XP flow.
Use active codes
Codes give Gems or Coins that can unlock items improving your efficiency. Always redeem before a session starts.

Vet Clinic basics (March 2026)

Maple Hospital added the Vet Clinic in March 2026. It requires Level 30 (Veterinarian) to treat animal patients. The Vet Clinic operates as a separate ward with its own treatment queue — you won't see animal patients in the General Ward.

For new players: skip the Vet Clinic until Level 30. The XP efficiency is lower than Doctor or Nurse, so it's worth exploring for variety rather than pure progression.

Server strategy — what to look for before joining

The single biggest XP multiplier in Maple Hospital is not your role or specialization — it is server population and role distribution. I tested this across 5 sessions at different server sizes and the difference is substantial.

Under 5 playersAvoid

Patient spawns are sparse. You will spend more time waiting for a new patient than treating. XP rate drops to effectively 2-4 XP/min for a Nurse. Only worth joining if you want a calm session for exploring the map.

6-10 playersBest for beginners

Enough patients to keep you busy without overwhelming the queue. Typically at least 1 experienced Doctor present to handle OR cases. You can practice all Nurse-tier illness types without competition for patients. Target this range for your first 3-5 sessions.

11-18 playersBest for XP grinding

Patient spawn rate is high and the illness queue includes more Severity 3-4 cases. Doctor and Surgeon roles are likely active, which keeps the OR chain running and increases downstream XP for specialist roles. This range is optimal once you have 10+ hours of Nurse experience.

19-25 players (max server)Advanced players only

Maximum XP potential but also highest complexity — Critical patients may back up if the Doctor-to-Nurse ratio is off. Useful for grinding after Doctor unlock (Level 10). As a new Nurse, the queue pressure in a max server can be disorienting before you have automated the treatment flow.
