Best-paying roles: Doctor vs Specialist vs Surgeon earnings
Role selection is the highest-leverage money variable after server population. Surgeon earns almost double a base Nurse — but requires Level 20 to unlock. I measured average payout per patient for each role across identical 20-minute windows in the same server, same evening session, to control for patient flow.
Surgeon
Surgical procedures include pre-op assessment, anesthesia administration, and the operation itself. Each step pays separately. In a full server on a Saturday evening, I averaged 18 patients in 60 minutes — that's the ceiling of what the OR can physically handle. The key insight: you earn more per minute waiting in the OR queue than you do completing faster Nurse tasks.
Doctor (Specialist)
Specialist consultations run 3–4 minutes and pay a flat $360 in my data. The Specialist bay sits adjacent to the OR, which means you catch surgical overflow — patients initially routed to Nurse who need escalation. This gives Specialist a higher effective patient count than a GP Doctor during busy hours.
Doctor (GP)
GP is the base Doctor role at Level 10. The $290/patient average covers standard consultations — diagnosis, prescription, referral. GP in the main ward has the broadest patient access, so throughput can match Specialist, but the lower payout per patient means your total is around 20% less in the same time period.
Nurse (L&D)
Labor & Delivery Nurse has higher patient volume than GP — 20-22 patients/hr in a good server because L&D patients process faster than surgical ones. But $210/patient is a hard ceiling without VIP. L&D is the best money option before Level 10, and competitive against GP Doctor even after you unlock it.

Method 2
Efficient patient routing to maximize patients per hour
The fastest players in any role are not faster at individual treatments — they position better. Here is the routing strategy I settled on after testing three different approaches across sessions 1 through 4.
Server population target: 15–20 players
At 14 players (Session 1), I averaged 42 patients in 3.5 hours. At 21 players (Session 5), I averaged 59 in 3.5 hours — a 40% increase in patient throughput from server population alone. Join servers with 15+ players minimum. Full 25-player servers can feel chaotic but the income data is consistently better.
Position near the OR corridor
The Specialist bay and OR share an adjacent corridor. A Surgeon positioned at the corridor junction intercepts patients who enter the surgical wing before they reach the standard Nurse or GP waiting area. In session 3, I tracked 67 patients in 4 hours partly because of position — intercepting walkthrough traffic reduced my dead-time between patients.
Skip low-complexity patients strategically
Standard check-in patients pay $80–120 (Receptionist range) even when treated by a Doctor. If your patient intake shows a low-complexity patient when the surgical queue is clear, passing that patient to a Nurse frees you to wait for a more valuable surgical escalation. This is a small optimization but at 15+ patients/hour, it adds up.
Play during peak hours (evenings / weekends)
The difference between a 14-player server and a 22-player server is roughly $4,000–6,000 over a 3-hour session. Peak hours on weekends regularly push servers to 20+. My best income session (Session 3: $32,160 in 4 hours) ran on a Friday evening. I use the in-game server browser to filter for higher player counts before joining.

Method 3
Daily missions and code bonuses
Daily missions are the highest-ROI activity in the game on a per-minute basis. Three missions completed in the first 30 minutes of a session pay $600–900, equivalent to doing 2–3 additional surgical procedures — but missions take less focused attention than high-value surgery routing.

Method 4
VIP and premium boosts: is the multiplier worth it?
VIP adds a 1.5× multiplier to all money earned from patient treatment. Missions are excluded from the multiplier in my testing. At Surgeon rates, VIP converts a $8,640/hr session to $12,960/hr — an extra $4,320 per hour.
VIP is worth it if...
- ✓You play 2+ hours per week consistently
- ✓You are already Surgeon or high-paying Doctor
- ✓You want access to premium wards (higher patient density)
VIP is less useful if...
- →You play under 1 hour/week (slow payback period)
- →You are below Level 10 — focus on role unlocks first
- →You primarily do admin / Receptionist tasks (low base)
Session 5 is my only VIP data point so far ($42,480 for 3.5 hours vs Session 2 non-VIP $24,480 for 3 hours). The 1.5× applied cleanly — my end-of-session earnings screen showed the multiplier visually confirmed before the total was credited.

Testing methodology
How We Tested Money Earning Data
All earnings figures come from 5 in-game sessions in May 2026 (Sessions documented above). Method: record starting wallet balance immediately after joining a server, record ending balance on exit, subtract to get session earnings. Patient count was manually tallied during play by noting each treatment completion. Server population was read from the in-game server browser on join. VIP status was confirmed on the earnings screen. No exploits, no external modifications — standard Maple Hospital game client only. All sessions played from Sydney, Australia on default Roblox client settings. Last verified: May 24, 2026.
