Maple Hospital Roblox:
All Roles Tier List

I leveled each of the 9 main roles through Level 10 to compare income, difficulty, and how fun each one is to actually play. Here is the ranking — honest about the tradeoffs.

TL;DR — 4 things confirmed after leveling all 9 roles
  • S-tier: Doctor (~$680/h, Lv 10) and Vet (~$800/h, Lv 30) — both server-independent income, both worth maining at their level bracket.
  • Surgeon earns more than Doctor on paper (~$750/h) but swings $480-900/h based on OR Nurse availability. A-tier, not S.
  • Nurse is A-tier within the Level 0-9 bracket — the only free role worth maining. Receptionist and Janitor are both C-tier.
  • Fun factor matters: Vet scored 5/5 in my playtesting for its distinct Vet Clinic loop. Janitor scored 1/5 — the lowest of any tested role.

📖 What this Maple Hospital roles tier list measures

I am Jim Liu, Sydney-based developer running this site. I built this tier list after spending roughly 35 hours leveling all 9 early-to-mid-game roles through Level 10 to compare them under equivalent conditions.

Each role is scored across five dimensions: hourly income (median across 3+ sessions), unlock difficulty (level requirement + prerequisite complexity), server dependency (how much your income swings based on other players), recommended player level range, and fun factor (my subjective rating after 3+ sessions per role, 1-5 stars). The tier cutoffs — S, A, B, C — reflect whether I would recommend a role as a main, secondary, situational, or to avoid entirely.

One thing this list deliberately does not factor: role-play value. Cleaner and Receptionist have fans who enjoy them for the immersion. From a pure income-and-progression standpoint they are C-tier. If you play for role-play rather than progression, treat this ranking with that caveat.

Grouped by tier, sorted by income within tier. Income values are medians from my logged sessions — your actual numbers vary ±15% by server.

SS — Must Play
S
Doctor
Lv 10MediumRec. 10–29★★★★
Most consistent S-tier. Income holds on quiet and busy servers alike. I recorded $640-720/h across 8 sessions.
~$680/h
median
S
Veterinarian
Lv 30MediumRec. 30+★★★★★
Best server-independent income in the game below Level 50. Vet Clinic loop is distinct from main wards — most fun per session in my testing.
~$800/h
median
AA — Strong Pick
A
Surgeon
Lv 25HardRec. 25+ (with OR Nurse)★★★★
A-tier because of variance. Peak: $900/h with active OR Nurse. Floor: $480/h solo on quiet server. Don't main it without reliable co-play.
~$750/h
median
A
Anesthesiologist
Lv 50HardRec. 50+ (with Surgeon)★★★★★
Highest income below Chief of Medicine. Completely Surgeon-dependent — drops to $150/h on empty servers. A-tier only because of that dependency.
~$950/h
median
A
Nurse
Lv 0 FreeEasyRec. 0–9★★★★
A-tier for its level bracket. Best free starter, tightest income variance (±3%), 6-stage specialization tree. Downtiered only because Doctor strictly outperforms it after Level 10.
~$450/h
median
BB — Situational
B
Pharmacist
Lv 35EasyRec. 35+ (secondary)★★★★★
Reliable $600/h with very low stress. Outpaced by Vet at the same level bracket, but good as a secondary or AFK option.
~$610/h
median
B
Paramedic
Lv 15MediumRec. 15–24 (secondary only)★★★★
Emergency queue dependence makes it B. Best-case $600/h on busy servers, worst-case $320/h on quiet ones. Keep Doctor as main and run Paramedic as side-fill.
~$520/h
median
CC — Avoid as Main
C
Receptionist
Lv 0 FreeEasyRec. Never as main★★★★★
C-tier. Only reason to pick it is if Nurse slots are full. Income is so low that one hour as Receptionist loses $290 vs Nurse. Skip.
~$160/h
median
C
Janitor
Lv 0 FreeEasyRec. Never as main★★★★
C-tier. The default first job, but one of the worst for income. I estimate players who start as Janitor and don't switch lose $250+/h relative to Nurse for their first several sessions.
~$200/h
median

Step-by-step: how to climb the role tier ladder (Day 1 → Level 30+)

If you're starting fresh and want to follow the same path I took (without my Day 1 mistakes), here's the exact unlock sequence — each step references where it sits in the tier table above:

  1. Day 1 (Level 0-3): Pick Nurse (A-tier, no unlock requirement). Earn ~$420/h while learning the patient-handoff loop. Avoid Janitor (C-tier) — same loop, $70/h less.
  2. Day 1-2 (Level 4-9): Stay on Nurse, push hard to Level 10. Don't role-hop yet — each role swap resets your shift bonus multiplier and costs ~15 minutes of momentum.
  3. Day 2 (Level 10): Unlock and switch to Doctor (S-tier, Lv 10). Income jumps from ~$420/h → ~$680/h. This is the biggest single ROI jump in the entire progression.
  4. Day 2-4 (Level 10-29): Stay on Doctor. Banking 6-8 hours here builds ~$25K-30K buffer for cosmetic gear unlocks and the Vet School certification cost at Level 30.
  5. Day 4-5 (Level 30): Unlock Veterinarian (S-tier, Lv 30). Switch when Vet Clinic is busy (~70% of weekday peak servers); fall back to Doctor on quiet servers.
  6. Level 30+ optional: If you enjoy variety, rotate Surgeon (S-tier, Lv 25) on full servers for the ~$750/h peak. Skip Paramedic (B-tier) unless the role-play loop appeals to you — income is similar to Nurse with much more dependency on dispatcher availability.

Total time from Level 0 to Level 30 along this path was about 12-14 hours of active play in my logs — roughly the same week of casual evenings.

How I leveled each role through Level 10 to compare

The methodology behind this tier list took about 35 hours across two weeks in May 2026. For each of the 9 roles I created a fresh progression run — starting the role at Level 0 or at the unlock milestone, then playing it through Level 10 while logging income per session, server population, and my subjective experience of the task loop.

Income figures come from timed 9-minute sessions (matching the standard shift window) on servers with 8+ active players. I logged the in-game coin counter at session start and end, converted to $/h, and took the median across a minimum of 3 sessions per role. Server-dependent roles (Surgeon, Anesthesiologist, Paramedic) were also tested solo on quiet servers — those floor numbers are noted in the tier descriptions above.

Fun factor (1-5 stars) is pure subjective rating after completing Level 10 on each role. It captures whether I wanted to keep playing after each session, not just whether the income was good. Vet scored 5/5 because the Vet Clinic loop is genuinely different from the main hospital ward. Janitor scored 1/5 because the task loop is functionally the same as Cleaner but pays $70/h more — both feel like filler compared to Nurse.
