I leveled each of the 9 main roles through Level 10 to compare income, difficulty, and how fun each one is to actually play. Here is the ranking — honest about the tradeoffs.
I am Jim Liu, Sydney-based developer running this site. I built this tier list after spending roughly 35 hours leveling all 9 early-to-mid-game roles through Level 10 to compare them under equivalent conditions.
Each role is scored across five dimensions: hourly income (median across 3+ sessions), unlock difficulty (level requirement + prerequisite complexity), server dependency (how much your income swings based on other players), recommended player level range, and fun factor (my subjective rating after 3+ sessions per role, 1-5 stars). The tier cutoffs — S, A, B, C — reflect whether I would recommend a role as a main, secondary, situational, or to avoid entirely.
One thing this list deliberately does not factor: role-play value. Cleaner and Receptionist have fans who enjoy them for the immersion. From a pure income-and-progression standpoint they are C-tier. If you play for role-play rather than progression, treat this ranking with that caveat.

Total time from Level 0 to Level 30 along this path was about 12-14 hours of active play in my logs — roughly the same week of casual evenings.

The methodology behind this tier list took about 35 hours across two weeks in May 2026. For each of the 9 roles I created a fresh progression run — starting the role at Level 0 or at the unlock milestone, then playing it through Level 10 while logging income per session, server population, and my subjective experience of the task loop.
Income figures come from timed 9-minute sessions (matching the standard shift window) on servers with 8+ active players. I logged the in-game coin counter at session start and end, converted to $/h, and took the median across a minimum of 3 sessions per role. Server-dependent roles (Surgeon, Anesthesiologist, Paramedic) were also tested solo on quiet servers — those floor numbers are noted in the tier descriptions above.
Fun factor (1-5 stars) is pure subjective rating after completing Level 10 on each role. It captures whether I wanted to keep playing after each session, not just whether the income was good. Vet scored 5/5 because the Vet Clinic loop is genuinely different from the main hospital ward. Janitor scored 1/5 because the task loop is functionally the same as Cleaner but pays $70/h more — both feel like filler compared to Nurse.
Doctor (S-tier, Lv 10) for Level 10-29. Veterinarian (S-tier, Lv 30) for Level 30+. Both earn $680-800/h with stable income across server types.
13 as of May 2026. This tier list covers the 9 roles between Level 0 and Level 50. Endgame roles (Specialist, Head Nurse, Chief of Medicine) are listed in the all-jobs tool but not scored here.
Surgeon earns more on peak servers (~$750/h vs Doctor's ~$680/h) but its income drops to $480/h solo. Doctor is more reliable as a main. Use Surgeon as a secondary once you have a consistent OR Nurse pairing.
Nurse — free at Level 0, ~$450/h, no server dependency, 6-stage specialization tree. Run Nurse as main until Level 10, then switch to Doctor.
